Jan Cochrane RN BSN M.Ed(DE)

With over 51 years dedicated to the nursing profession, I have been fortunate to gain experience across a wide range of care settings. As a Certified Diabetes Educator, I supported clients in their self-management journeys, delivered diabetes education to healthcare professionals, and collaborated with First Nations Communities. My work in home care included direct client support, advisory roles in First Nations settings, and, more recently, instructing the Home Health Aide course at Suncrest College.

At the national level, I served on the Diabetes Canada Board, representing both health‑care professionals and people living with diabetes to advance high‑quality care across the country. I also collaborated with the National Aboriginal Diabetes Initiative, supporting the rollout of funding and community‑based programs.

Presently, I serve as a Social Prescriber Community Connector in Yorkton, Saskatchewan, with a focus on meeting the needs of seniors. Through this position, I have witnessed the significant disparities and service gaps faced by older adults, especially in relation to health determinants and social isolation.

I dedicate my time to volunteering with the local arts council, the Legion, and an exercise group for older adults. Above all, I cherish my role as “Nanny the Nanny” to my grandchildren. In addition, I find joy in baking, snowshoeing, hiking, kayaking, and performing with the city band.
